The present review aims to revisit this important topic and will detail the recent advances in the pathophysiology and management of tinea corporis, tinea cruris, and tinea pedis while highlighting the lack of clarity of certain management issues.ĭermatophytes are the most common agents of superficial fungal infections worldwide and widespread in the developing countries, especially in the tropical and subtropical countries like India, where the environmental temperature and relative humidity are high. Updated Cochrane reviews on the use of topical therapy in tinea corporis, cruris, and pedis, and few on oral therapies have helped to bridge this knowledge gap but still well-designed trials, national and/or international evidence-based guidelines and recommendations on the dose and duration of the use of systemic antifungals in tinea corporis/cruris are conspicuous by their absence. The more recent guidelines published by the British Association of Dermatology and in the British Medical Journal have largely focused on tinea capitis and tinea unguium with scarce reference to tinea corporis/cruris. In fact, one has to go back nearly two decades to find guidelines on the management of tinea corporis and cruris (by the American Academy of Dermatology), and these at best, appear inadequate in today's world. Other clinical variants include tinea imbricata, tinea pseudoimbricata, and Majocchi granuloma.ĭespite the increasing prevalence of cutaneous dermatophytosis across the world, and especially in tropics, research in this area has often been neglected. Finally, based upon the affected site, these have been classified clinically into tinea capitis (head), tinea faciei (face), tinea barbae (beard), tinea corporis (body), tinea manus (hand), tinea cruris (groin), tinea pedis (foot), and tinea unguium (nail). Based upon mode of transmission, these have been classified as anthropophillic, zoophilic, and geophilic. Based upon their genera, dermatophytes can be classified into three groups: Trichophyton (which causes infections on skin, hair, and nails), epidermophyton (which causes infections on skin and nails), and Microsporum (which causes infections on skin and hair). The present review aims to revisit this important topic and will detail the recent advances in the pathophysiology and management of tinea corporis, tinea cruris, and tinea pedia while highlighting the lack of clarity of certain management issues.ĭermatophytes are fungi that invade and multiply within keratinized tissues (skin, hair, and nails) causing infection. However, due to the lack of updated national or international guidelines on the management of tinea corporis, cruris, and pedis, treatment with systemic antifungals is often empirical. The last few years have seen a significant rise in the incidence of chronic dermatophyte infections of skin which have proven difficult to treat. Management involves the use of topical antifungals in limited disease, and oral therapy is usually reserved for more extensive cases. ![]() Several new techniques such as polymerase chain reaction (PCR) and mass spectroscopy can help to identify the different dermatophyte strains. Diagnosis, though essentially clinical should be confirmed by laboratory-based investigations. Hence, a lack of delayed hypersensitivity reaction in presence of a positive immediate hypersensitivity (IH) response to trichophytin antigen points toward the chronicity of disease. Recent developments in understanding the pathophysiology of dermatophytosis have confirmed the central role of cell-mediated immunity in countering these infections. ![]() The prevalence of superficial mycotic infection worldwide is 20–25% of which dermatophytes are the most common agents. ![]()

![]() ![]() The block ends with attempting a one rep max that is hopefully a PR. As in intensification lock #1, volume gradually decreases each week as intensity ramps up after a somewhat tame week 1.ĪMRAPs are employed again to help keep intensity high and to give you the opportunity to set rep PRs if you feel they’re there. Most notably: the competition deadlift, which does not appear in either of the previous blocks (as they’re currently written, at least – no one says you couldn’t make the competition deadlift a main lift for either of those blocks). The second intensification block is structurally similar to the first, but includes many different lifts. A successful volume block is necessary for setting PRs in the following intensification block #2. There is a pretty significant amount of accessory volume in this stage as well, which should be helpful for triggering hypertrophy and aiding the main lifts.Įmpire Barbell stresses the importance of hitting clean reps during this training block, taking a bit of weight off of the bar if necessary. Intensity doesn’t exceed 75% of 1RM throughout the block, while working sets increase each week, culminating in 10 working sets for the squat and bench press in week 6. The volume block, designed to be run after the intensification block, dials back intensity and significantly increases the number of working sets and rep ranges. ![]() Power shrugs are, essentially, rack (or block) pulls with a whole lot of trapezius engagement at the top. From there, intensity and volume increase in week 2 before volume dials back in weeks 3 and 4 as the intensity increases each week in a fairly traditional “linear” fashion.Įmpire Barbell does not recommend running this block more than twice in a row before switching blocks or changing the main movements of the block. As the barbell flies upward, shrug hard at the top and allow your elbows to bend slightly. This block starts off with low-ish volume and middling intensity during week 1, which acts as either a deload or an introductory week, depending on whether you’re running this block for the first time or repeating it. The organization of your written work includes two elements: (1) the order in which you have chosen to present the different parts of your discussion or argument, and (2) the relationships you construct between these parts. For help in this area (and a more thorough explanation of the “reverse outlining” technique described in the previous paragraph), please see the Writing Center’s handout on organization. If after doing this exercise you find that you still have difficulty linking your ideas together in a coherent fashion, your problem may not be with transitions but with organization. This exercise should help you to see the order of and connection between your ideas more clearly. In the margins of your draft, summarize in a word or short phrase what each paragraph is about or how it fits into your analysis as a whole. Since the clarity and effectiveness of your transitions will depend greatly on how well you have organized your paper, you may want to evaluate your paper’s organization before you work on transitions. You are working on a group paper the draft you are working on was created by pasting pieces of several people’s writing together. You wrote your paper in several discrete “chunks” and then pasted them together. You tend to write the way you think-and your brain often jumps from one idea to another pretty quickly.Your readers (instructors, friends, or classmates) tell you that they had trouble following your organization or train of thought.Your instructor has written comments like “choppy,” “jumpy,” “abrupt,” “flow,” “need signposts,” or “how is this related?” on your papers. How can you tell whether you need to work on your transitions? Here are some possible clues: ![]() Signs that you might need to work on your transitions In providing the reader with these important cues, transitions help readers understand the logic of how your ideas fit together. They are words with particular meanings that tell the reader to think and react in a particular way to your ideas. Transitions are not just verbal decorations that embellish your paper by making it sound or read better. Transitions signal relationships between ideas-relationships such as: “Another example coming up-stay alert!” or “Here’s an exception to my previous statement” or “Although this idea appears to be true, here’s the real story.” Basically, transitions provide the reader with directions for how to piece together your ideas into a logically coherent argument. Whether single words, quick phrases, or full sentences, they function as signs that tell readers how to think about, organize, and react to old and new ideas as they read through what you have written. In other words, transitions tell readers what to do with the information you present to them. Transitions help you to achieve these goals by establishing logical connections between sentences, paragraphs, and sections of your papers. In both academic writing and professional writing, your goal is to convey information clearly and concisely, if not to convert the reader to your way of thinking. ![]() The function and importance of transitions This handout will introduce you to some useful transitional expressions and help you employ them effectively.
